Create Your First Registration Form

This guide shows you how to create your first B2B registration form so wholesale customers can submit a signup request from your storefront.

A registration form collects information from shoppers who want to become wholesale customers (company name, contact, business type, tax ID, etc.). Each submission is stored in the app for you to review and approve manually.

Registration submissions are not automatically turned into Shopify B2B companies. They are signup requests awaiting your review.

  1. In the app, go to Registrations.
  2. Click New.
  3. A default template loads pre-filled with common wholesale fields (Company name, Business type, EIN, Contact person, Email, Phone, Address, Country, Number of employees, Notes).

The editor has three panels:

  • Settings (left): General info, Style, Submit, Emails.
  • Structure (middle): drag-and-drop field list.
  • Preview (right): live preview of the form.

Available field types:

  • Text, Email, Phone, Number, Date
  • Dropdown, Radio, Checkbox
  • Address, Country
  • Text block, Divider, Spacer

Drag a field from the structure panel into the form, or click a field to edit its label, required setting, and options.

Under the Submit and Emails tabs:

  • Customize the submit button text.
  • Set the recipient email that receives new submission alerts (your shop owner email is pre-filled by default).
  1. Click Save.
  2. In the registrations list, toggle the form Enabled so it can receive submissions.

Pick one of three ways to place the form on your storefront:

Submitted signups appear under Registrations with a submissions count per form. Review each one and contact the applicant to finish onboarding as a B2B customer.
